The outcomes of clinical studies to reduce atherosclerotic cardiovascular disease (ASCVD) by lowering plasma TG levels have been disappointing.1,2 Nevertheless, numerous novel TG-lowering strategies are currently being tested in the clinic, though the aims have shifted toward reducing the risk of pancreatitis in patients with severe hypertriglyceridaemia (sHTG).3 The question is whether there is still room for these drugs in the cardiovascular arena. This commentary will focus on the intriguing effects of increasing lipoprotein lipase (LPL) activity on lipoprotein metabolism when targeting its protein inhibitors APOC3, ANGPTL3 and ANGPTL4 in humans.
